These brand new locations will continue to offer primary care, mental health, and social services for people of all ages. Please see below for a list of the providers seeing patients at each location. If you do not see your provider’s name, please call us at (844) 434-2778.
The only thing changing is our location! As always, we are committed to providing you with outstanding care. Your medical record will be available at the new location. We will continue to accept your insurance plan. If you do not have insurance, we can help. You will not be turned away because you cannot pay.
If you prefer to transfer to another Institute for Family Health center, please make an appointment at any of our centers. Please note, the Sidney Hillman / Phillips location at 16 East 16th Street will close permanently on December 1, 2017.
